Hospital advisory team	Yes		Monday to Friday 9-5 with an emergency visiting service Saturday – Sunday 9-5. Advisory service. Criteria include patients with life limiting illness and a specialist palliative care need such as uncontrolled symptoms, psychological distress, complex social issues. Medical consultants – Sarah Kelt, Claire Hookey Nurse consultant – Jane Thompson-Hill 7 CNS team members

Outpatient clinics	Yes		Tuesday afternoon clinic provided by medical consultant. Referrals accepted from within the trust and other palliative care providers.
