Hospital Palliative Care Team is available from 8.30am -4.30pm Monday to Friday. The team consists of a Consultant in Palliative Medicine who is available for a half session daily, Monday - Friday, 2 Macmillan Clinical Nurse Specialists, 2 Macmillan Occupational Therapists, 1 Nurse End of Life Care facilitator. Extended members of the team include Chaplaincy and Social Work input. Referral to the team is by telephone from the community, paper referral from the wards with the criteria for referral outlined. These include pain and symptom management, complex discharge planning and functional assessment, spiritual support, information and advice with regard to diagnosis and treatment, carer support, and assessment for Preferred place of care. |
