Key Worker Cards
As part of the Supportive & Palliative Care Improving Outcomes Guidance the Network have been working with all Network Site Specific Groups to produce Key Worker Cards for all Cancer Patients.
The Key Worker is a Health Professional who takes a key role in the co-ordination and promotion of continuity of Patient care. Each cancer Patient will be informed of their key worker at diagnosis and the key worker card will be issued which will contain important contact details of all health professionals involved in the patients care.
